Job Description
Job Title: Case Manager Scattered Site Oversight
Location: New York City (5 Boroughs)
Position Summary: $68000 to $70000
The Case Manager Scattered Site Oversight will be responsible for monitoring supporting and ensuring quality service delivery by an external case management services provider assigned to a scattered-site transitional housing program for homeless individuals and families. The programs housing units are located throughout the five boroughs of New York City. This position does not involve direct case management but instead focuses on oversight coordination and performance evaluation of contracted case management teams.
Key Responsibilities:

Serve as the primary liaison between the program and the external case management services provider.

Monitor performance and compliance of the external provider with contractual obligations service delivery standards and regulatory requirements.

Conduct regular site visits and file reviews to ensure quality of service appropriate documentation and timely interventions.

Provide coaching feedback and technical support to contracted case managers as needed.

Coordinate with housing specialists shelter staff legal service providers and community-based organizations to ensure continuity of care.

Analyze data and service outcomes to identify gaps in service or performance trends; recommend and implement corrective actions when needed.

Facilitate regular case conferencing performance reviews and quality assurance meetings with the external provider.

Maintain a working knowledge of OTDA and NYC DHS regulations related to transitional housing and case management.

Assist with preparing reports and documentation for audits funding sources and program evaluation.
Qualifications:

Bachelors degree in Social Work Human Services Psychology or a related field; Masters degree preferred.

Minimum 3 years of experience in case management with at least 1 year in scattered-site or community-based housing programs.

Prior experience overseeing or coordinating contracted or third-party service providers strongly preferred.

Familiarity with OTDA NYC Department of Homeless Services or similar regulatory agencies and reporting systems.

Excellent organizational analytical and written/verbal communication skills.

Ability to travel across the five boroughs as needed.

Bilingual (English/Spanish) preferred but not required.

Convene and lead case conferencing and performance review meetings with the external case management team to review service progress and address challenges.
