Job Description
We are seeking a fulltime Peer Support Specialist to clinical services to all clients receiving ACT team services including but not limited to: service coordination including working with the ACT team leader to monitor each individuals clinical status and response to treatment; symptom assessment and management. The Certified Peer Support Specialist also provides essential expertise and consultation to the entire team to promote a culture in which each individuals point of view and preferences are recognized understood respected and integrated into treatment rehabilitation and community selfhelp activities.

Location: Charlotte NC
We are seeking a fulltime Peer Support Specialist to provide the following services:
- Assist in identifying planning scheduling and implementing services for the client.
- Assist in the development of transition plans when clients move in and out of services.
- Act as an advocate in the community.
- Provide any written documentation as required. This will include treatment notes goals and plans for the medical record (e.g. service notes input into personcentered plan etc.
- Be available to respond as needed to clients and coworkers 24/7/365.
- Demonstrate fidelity to ACT Model as measured by TMACT.
