Role Responsibilities:
- Coordinate both formal and informal supports
- Manage clients with complex needs and handle crisis situations
- Collaborate with a Multidisciplinary Panel
- Engage with Mental Health Justice System Housing and Health sectors
- Undertake clerical administration and report writing
- Network and communicate with key stakeholders
- Work independently either from home or onsite.
- Work in a sensitive complex and demanding environment with a high degree of autonomy and selfmanagement.
Qualifications & Experience:
- Minimum of Certificate IV in Community Services or higher (or a related discipline and/or relevant experience and knowledge.
- Two or more years experience working with participants with any of the following: Mental health (moderate to complex) high medical needs behavioural support needs
- Experience in Forensic Disability or Forensic Mental Health is a plus
- Demonstrated knowledge of the NDIS and experience working with its participants
- Valid Drivers License with a suitable insured vehicle
- A current Working with Vulnerable People registration
- Recent police check (or willingness to obtain one).
