Description

Position Summary:

Reporting SR Department Director the community resource/Job development coordinator responsibilities include engaging community partners to expand both the volunteer and employment opportunities for the individuals at Heartshare. Additional to this is working with DOE transition coordinators and care mangers to expand the Employment programs at Heartshare. Internally at Heartshare to work with the individuals and families at our Day Hab programs and residence who are interested in pursuing employment opportunities and to assist in training staff for Pathways to employment.

Primary Responsibilities and Essential Functions of Position:

Program Operations

  • Employment and Volunteer Site Development
    • Collaborate with Day and Employment programs to identify and develop a variety of communitybased employment and volunteer opportunities
    • Identify and follow up on job openings
    • Build and maintain job and volunteer site relationships with other communitybased sites/contacts through periodic site visits and regular phone contacts.
    • Provide assistance so day programs can independently develop additional volunteer sites based on the needs of their programs
    • Partner with specific day program staff to provide expertise in how to follow through on volunteer sites that have been developed.
    • Perform job and volunteer site and examine task analysis whenever a new employment or volunteer opportunity is developed to ensure safety
    • Customize potential job and volunteer placements to fit the needs of the employer and the individuals skill set
  • Business and Employer Engagement:
    • Cultivate leads for the purpose of establishing relationships with businesses and non profit organizations for the purpose of creating an employment and volunteer pipeline between HeartShare and other entities.
    • Cultivate business partnerships in the community.
    • Collaborate with HeartShare St. Vincents and HeartShares Education division to explore possible collaborations.
    • Maintain close working relationships with employers/business associations
    • Develop partnerships via forums and conferences that benefit HeartShare and the Day and Employment Programs
    • Educate employers about the benefits of hiring people with developmental disabilities
    • Educate employers on the various services HeartShare provides to the community
    • Assist jobsites with completing appropriate documents in order to benefit from incentives
    • Schedule interviews and follow up with potential employers and volunteer providers to negotiate customized placements and paid employment.
    • Promote employment services to area businesses to create future partnerships
    • Network with the local arts and business communities as well as various community groups (Elks and Kiwanis clubs etc) to explore and create employment opportunities and meaningful activities that benefit HeartShare the day and employment programs.
    • Participate in OPWDD annual required and other training
    • Perform other employment related duties as needed. eg. Job Coaching
    • Expand Employment program into Queens

  • Day and Employment Program Enrollees Engagement:
    • Facilitate job readiness and soft skill training workshops including interviewing skills resume development work habits and computer literacy.
    • Meet with individuals to discuss job opportunities assist with scheduling interviews and provide post interview feedback.
    • Identify prepare and confirm job ready individuals for interviews/job placement provide assistance with obtaining proper ID and documentation for the interview/hire.
    • Provide career counseling and other employment related information
    • Meet with program staff and individuals to identify and develop appropriate enrichment activities for the individuals who participate in the day and employment programs.
    • Provide resume and cover letter development.
    • Share employment related information with the day and employment program staff individuals families/guardians and others as
    • Maintain a Job bank and binder of potential community based activities and resources available for individuals with in the day and employment programs

Documentation and Fiscal

  • In accordance with the agencys policy secure documents for submission including but not limited to Scripts Informed Consent Forms PPT sign offs etc.
  • Along with the Employment team review Peare to ensure that services are documented as
  • Timely submission of monthly report to OPWDD QA and other required reports are uploaded to Peare and Sharedrive
  • Has system in place to ensure all required paperwork is completed on time including daily notes monthly reports and other reports as assigned. eg. OPWDD Discovery and Vocational Assessment reports
  • Maintains documentation of placement services rendered and employment verifications as necessary.
  • Completes internal statistical and progress reports of funding sources

Quality Assurance

  • Along with the Employment staff ensure that programs meet and exceed all regulatory standards and that all state and federal regulations as well as HeartShares internal policies and procedures are followed.

Staff Supervision and Development

  • Participating in the training of both Day Hab and Employment staff as needed on regulations for SEMP Pathways to employment and Pre Vocational programs

Other

  • Reports all incidents to program management immediately upon discovery as per OPWDD and rustic center requirements
  • Represents Heartshare both within and outside the organization in a professional manner that promotes the mission vison and values of the agency and positively reflects Heartshare.
  • Abides by the agency dress code as well as any dress codes required by a given community site including business attire when interacting with companies.
  • Adheres to all Heartshare policies and procedures
  • Undertakes any additional tasks or initiatives as assigned

Qualifications

Minimum Education: High school diploma or GED.

Preferred Education: Bachelors degree from four year college or equivalent.

Minimum Experience: Oneyear experience working with individuals with developmental disabilities.
