Description

The TRIA LPN Clinical Navigator will oversee daily clinic flow through Checkin and Copay collection processes standard rooming history and evaluation assist with documentation clinical procedures after visit education scheduling of followup appointments as well as facilitating overall patient care needs and coordination of care across the enterprise for our patients and families.

This role will also be responsible for ensuring medical orders/care are delivered as delegated by providers and registered nurses to support patient care. Quality of care and exceptional customer service are always delivered to our patients and families.

Work Schedule:

1.00 MF Days

Required Qualifications:

  • Graduate from an accredited school of practical nursing
  • Must maintain BLS through AHA or ARC online version not accepted
  • LPN must maintain a current state license of practical nursing (MN & WI).
  • Strong clinical and human relation skills are required.
  • Ability to effectively communicate both orally and in writing and maintains standards of clinical skills and professional ethics as established by the Nurse Practice Act and PNHS.
  • Basic computer/keyboarding/mouse skills required.
  • Demonstrates knowledge of agerelated growth and development principles necessary to provide appropriate service and ensure safety of patients.
  • Can make autonomous decisions regarding patient care related issues and be able to make independent decisions based on scope of practice and protocols established with their physician.
  • Excellent interpersonal listening critical thinking and organizational skills.
  • Team player that collaborates to achieve highest outcomes.
  • Safely and effectively operate the following: Various Health Testing Equipment (depending upon specialty area) Personal Computer Photocopier Printer FAX Machine and Telephone.
  • Proficient in word processing electronic medical records ECG dynamap defibrillator and various Orthopedic equipment/braces/splinting.
