Job Description

CPSP Coordinator/Educator


$25.00 $31.00 per hour

SUMMARY: The Comprehensive Perinatal Services Program (CPSP) Coordinator/Educator will coordinate with a multidisciplinary provider team in the delivery of Comprehensive Perinatal Services in a Federally Qualified Health Center setting. The CPSP coordinator is responsible for navigating and supporting patients through their obstetric care and CPSP service components: psychosocial and nutrition health education client orientation initial assessment individualized care plan including intervention and reassessments and postpartum assessment and care. The goal for the CPSP coordinator/educator is to improve perinatal access to care and pregnancy outcomes through enhanced prenatal care.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

1. Supports and implements the organizations vision mission and values.

2. Performs all job functions in a professional courteous and timely manner. This includes answering all phone calls emails and providing excellent customer service to internal and external customers.

3. Fosters and promotes a culture of service excellence and selfaccountability.

4. Responsible for implementations and integration of CPSP Program including nutrition psychosocial and health education assessments interventions and perinatal education with basic obstetrical care.

5. Responsible for ensuring the integrity and quality of the health services delivered in Comprehensive Prenatal Services and in compliance with Federal State and SD County Standards and individual site policies procedures and protocols.

6. Works cohesively with pediatric and dental departments to ensure patient and newborn care coordination.

7. Maintains all program logs and statistics for internal and external review and reporting.

8. Provides followup and completes proper documentation of postpartum delivery and lactation education.

9. Follows up on relevant laboratory tests.

10. Participates in Continuous Quality Improvement plans for site and program.

11. Establishes rapport with WIC and external agencies to aid in patient support.

12. Provides support to OBGYN provider(s) as needed.

13. Performs all other duties as assigned.

14. Keeps up with all necessary CPSP certifications.

15. Must be willing and able to work at all locations as needed to meet patient care needs. 16. Must be willing and able to work all business hours including evenings and weekends.

4. Bilingual in English and Spanish mandatory

9. Adapts well to changes in work requirements.

10. Performs work in an honest and ethical manner.

LANGUAGE SKILLS: Ability to read and interpret documents such as policies and procedures benefits information board minutes routine mail simple contracts and instruction manuals. Ability to compose routine reports and correspondence. Ability to speak effectively with employees visitors and management.

REASONING ABILITY: Ability to exercise common sense in carrying out instructions furnished in written oral or diagram form and in other daily situations that arise. Ability to deal with problems involving several concrete variables in standardized situations. Ability to make decisions and execute them in a timely manner in order to produce a positive outcome.

OTHER SKILLS AND ABILITIES:

1. Performs as a selfstarter who needs no reminders of duties and tasks that need to be completed.

2. Ability to organize and prioritize work with minimum supervision.

3. Proficiency with computer applications such as email use of webbased portals the Microsoft Office Suite (Excel Outlook Word and PowerPoint) and Electronic Medical Record Systems.

The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 30 pounds. The employee is occasionally required to ascend and descend one flight of stairs. Specific vision abilities required by the job include close vision color vision and the ability to adjust focus.

QUALIFICATIONS: To perform this job successfully an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. EDUCATION/EXPERIENCE:

(A) A Masters Degree (or higher) in Community or Public Health Education form a program accredited by the Council on Education for Public Health and who has one year of experience in the field of Maternal and Child Health or

(B) A Baccalaureate Degree with a major in Community of Public Health Education and who has one year of experience in the field of Maternal and Child Health.

CPSP certification

One year of Obstetrics experience

BLS/CPR certification

Bilingual Spanish

Salary Description
$25.00 $31.00 Per hour
