Clinical Case ManagerResidential Program (Adult) 14CM3
Gilead Community Services is seeking an attentive and recoveryoriented Clinical Case Manager to join our multidisciplinary team in our supervised apartment program for women.
About the position:
The Clinical Case Manager for Gileads Middletown area adult supervised apartment program works Sun Thurs 3p11p 40 hours)providing case management advocacy educational and supportive services in a program serving women with major mental illness cooccurring substance use disorders and histories of significant trauma. The individual in this role carries out all phases of clinical services including intake engagement assessment treatment planning case management referral and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning.
Minimum Requirements:
- Bachelors degree in a human service field; Masters degree preferred
- One year of clinical experience
- Must maintain and provide proof of valid drivers license and automobile insurance in good standing
- Reliable use of personal vehicle as needed
- Experience with trauma treatment and cooccurring psychiatric and substance abuse disorders preferred

This is a union position and pays $18.8920.69per hour dependent on level of education.
Responsibilities:
- Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and education
- Working with the treatment team to manage clients mental health and assessing for risks implementing risk management plans as necessary
- Providing recoveryoriented clinical casemanagement services with an emphasis on dignity respect and inclusion of family and other natural supports
- Providing support skills training and education in areas including daily living skills nutrition financial management and symptom management
- Monitoring positive outcome measures and ensuring program compliance through accurate and timely data reporting
- Completing assessments treatment plans and other documentation electronically and on paper in a timely manner
- Acting with compassion integrity and ethics
