Job Description

Status: FullTime HourlyFLSA Classification: NonExempt

Summary of the Position:The primary responsibility of the Case Manager/Advocate is to initiate organized plan develop and implement direct case management and advocacy services to assigned clients and their families as part of theMerrimack County Intensive Service Option (ISO) InHomeprogram.

  • All service plans are based on strengthbased approach using the wrap around model.
  • The Advocate maintains a caseload of no more than 10 families per month on average and is required to provide a minimum number of facetoface hours with families each week. Documentation must be completed in accordance with New Hampshire Medicaid standards.
  • Applicant must be dependable committed and able to serve as a positive role model for youth in community school and home settings.
  • The position offers flexible hours competitive weekly pay and activity reimbursement.

Qualifications/Requirements: Bachelors degree in social work psychology education or a related field with an emphasis in human services and two years of experience working with children and families

  • Demonstrated communication skills both oral and written. Experience in the provision of home and communitybased services.
  • Position requires reliable transportation valid drivers license and current automobile insurance coverage.
