Job Description

We are working closely alongside a Local Authority in Merthyr Tydfil to assist with the appointment of a Support Worker on a 6month contract likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: 13.43 16.71 per Hour

Summary:

The Personal Advisor will provide a needsled support service for young people aged 16 preparing to leave care and those in postcare transition. The role ensures pathway plans are tailored to individual needs focusing on health education financial stability and independent living. The advisor will act as an advocate offering direct support facilitating group sessions and coordinating with key stakeholders to promote positive outcomes for young people.

Responsibilities:

  • Young Peoples Support & Advocacy: Provide quality support for young people aged 16 ensuring their needs are met in accordance with the Social Services and Wellbeing Act 2014 and the Children (Leaving Care) Act 2000. Act as an advocate ensuring their rights and entitlements are understood and upheld. Maintain regular contact with young people offering guidance on education training employment and independent living skills.
  • Pathway Planning & Case Management: Take case responsibility for post18 young people ensuring pathway plans are reviewed every six months and updated as required. Assist young people in assessing their needs ensuring pathway plans include clear measurable objectives. Support the transition process helping young people move into stable accommodation and develop independent life skills.
  • Financial Management & Support: Assess financial needs ensuring allowances benefits and emergency payments are allocated correctly. Organize payments using the 16 financial systems in line with Merthyr Tydfil County Borough Councils policies. Assist young people in managing their settingup home allowances offering financial literacy guidance to support longterm stability.
  • Accommodation & Social Inclusion: Maintain positive relationships with accommodation providers including supported lodgings to ensure young people have access to safe and appropriate housing. Facilitate group sessions and oneonone support to improve life skills promote peer support and relieve social isolation.

Qualifications:

  • Qualification in Social Work Youth Work or a related field (desirable).

Experience:

  • Experience working with young people particularly those leaving care.
  • Strong knowledge of Children (Leaving Care) Act 2000 and Social Services and Wellbeing Act 2014.
  • Experience in case management and pathway planning.
  • Experience supporting education training and employment pathways.

  • Communication & Advocacy: Ability to communicate effectively and advocate for young peoples rights.
  • ProblemSolving: Strong ability to assess needs and develop creative solutions to challenges.
  • Relationship Management: Build trusting relationships with young people and stakeholders.
  • Resilience & Adaptability: Work in a fastpaced flexible environment adapting to young peoples evolving needs.

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires a basic DBS
