As an RN Care Manager your ability to manage patient care and provide services requires substantial specialized bedside nursing skills for symptom management crisis intervention and family education. In addition to excellent written/verbal communication problemsolving and decisionmaking abilities the candidate should demonstrate experience with developing and maintaining rapport with patients and families and the ability to work well with an interdisciplinary team.

• Graduate from an accredited school of nursing
• Currently licensed to practice nursing by the Virginia State Board of Nursing
• Two years experience in acute care home health hospice med/surg or geriatric nursing
• Current Virginia drivers license and driving record that meets the standards outlined in current policies.
