Shift Type/Shift Times/Hours/Day: Days8:00 AM 4:30 PM 8

Shift Notes: MF 8:00 AM4:30 PM 8 hour days no call no weekends 40 hour guarantee

Travel Policy: The local traveler policy is to reduce the travel rate by 10%. Anyone with a permanent address within a 35mile radius of the facility is considered a local traveler.

Further Specifics:

  • Nonbillable Hours: 12 hours of initial hours
  • OT/Holiday/Call back Markups: 1.1x Regular rates
  • Extension Discount: $1/hr on the initial extension.
  • Position Summary: The occupational therapist accurately assesses and identifies deficits in a patients ability to perform activities of daily living (ADL) and instrumental activities of daily living (IADL) relative to physical cognitive and/or psychological impairments. Develops an individualized treatment plan with patientcentered goals that will be accomplished by therapeutic interventions and the use of ADLs. This may include adaptation or modification of the environment or objects within the environment to facilitate engagement and participation.

    Essential Functions and Responsibilities:

    1. Performs patient evaluations by establishing an occupational profile analyzing occupational performance and collaborating with the patient to establish goals geared toward the experiences values priorities and desired outcomes of the client. This process may also include performing relevant systems reviews and selecting and administering specific tests and measures to obtain data.

    2. Establishes an individualized plan of care and selects therapeutic interventions that address the specific impairments identified in the evaluation and following physician orders.

    3. Conducts ongoing reassessment of the patient and their response to treatment.

    4. Initiates and manages the discharge plan beginning at the first visit and continuing throughout treatment.

    5. Supervises and delegates to ancillary staff and students consistent with state laws and department policy.

    6. Completes legible accurate and clear documentation within the established timeframe identified in the department policy and procedure and following accreditation requirements and insurance regulations.

    7. Communicate with the patient family caregivers significant others and members of the health care team to promote maximum benefit of care utilizing AIDET.

    8. Assesses the educational needs of the patient family caregiver or significant other and provides education to meet those needs.

    9. Participates in professional growth and development through participation in educational programs including inservice meetings and workshops and by reading current literature concerning the scope of practice.

    10. Actively participates in quality improvement activities.

    11. Provides an environment conducive to safety for patients visitors and staff. Complies with appropriate and approved safety standards.

    12. Functions within the scope of practice as defined by the states practice act.

    13. Performs other related duties as required and directed.

    SUBMISSION REQUIREMENTS

    Required Qualifications:

    Graduate of an accredited program of occupational therapy.

    Current State Board of Occupational Therapy licensure.

    BLS certification.

    Preferred Qualifications:

    One year of occupational therapist experience.

    Registration through the accrediting body National Board Certificate for Occupational Therapy (NBCOT).

    • References NOT required.
    • A skills checklist completed within one year of application for RN specialties is required. All other professions outside of Nursing are not required but preferred.
    • Michigan license verification is required for nurses. Please provide LARA and Nursys; if there is a license hit it would be in the travelers best interest to provide a writeup of what happened for the managers review.
    • Physical copies of certifications are only required in the profile if listed above.
    • Fulltime employees are not eligible for contract hire within six months of employment.
