ABOUT: We are a 25 bed multispecialty medical clinic and rehabilitation center located in beautiful rural Craig CO. Located just 45 miles west of Steamboat Springs CO Ski Resort. Craig offers many opportunities for local outdoor recreation camping rafting paddle boarding kayaking skiing/snowboarding mountain biking hunting fishing etc.We provide L4 ED care inpatient care diagnostic imaging surgery laboratory special care unit cardiopulmonary oncology and chemotherapy services and more.SHIFT: Three 12 hour shifts 36 hours guaranteedCERTS: BLS Active Colorado Physical Therapy License or compact license.EMR: EPICHOUSING: Leads uploaded into our Medefis account under facility group documentsPosition Purpose: To coordinate and implement an individualized plan of care for each patient using the nursing process practicing within their scope standards policies values and mission of the organization.Essential Functions and Basic DutiesPosition Specific Performance Expectations: Plans implements and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Monitors records and communicates patient condition as appropriate. Collaborates as needed across disciplines to coordinate patient care including patient transfer discharge referral and spiritual/psychosocial support needs. Identifies barriers to effective communication based on psychosocial literacy financial and cultural considerations. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age culture condition and circumstances. Effectively delegates patient care to ancillary personnel. May precept student nurses and new hires. Employs critical thinking skills decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast paced environment and take appropriate action. Performs other duties as assigned. Partners with the healthcare consumer to implement a patient centered plan of care. Demonstrates caring attitudes and behaviors to develop therapeutic relationships. Uses evidencebased interventions and strategies to achieve optimal outcomes. Integrates critical thinking and technology solutions. Delegates according to the health safety and welfare of the patients we serve. Demonstrates respect equity and empathy in actions and interactions with culturally diverse and vulnerable patients. Organization Wide Performance Expectations: Demonstrates commitment to performing according to the CHOICE values of MRH and representing the organization in a positive and professional manner. Maintains patient confidentiality at all times. Upholds regulatory requirements to ensure continual compliance with departmental hospital state and federal regulations and policies. Follows all policies and procedures to ensure a safe environment for patients public and staff. Completes annual education training inservice and licensure/certification requirements; attends departmental and organizational staff meetings or reads meeting minutes. Reports to work on time and as scheduled; completes work within designated timeframes. Establishes and maintains effective verbal and written communication and good working relationships with all patients staff and vendors. Utilizes initiative; strives to maintain steady level of productivity; selfmotivated; manages activity and time. Actively participates in departmental and facility performance improvement and continuous quality initiatives. QualificationsMinimum Requirements: Must be at least 16 years of age (21 for driving positions) Must be able to legally work in the United States Must be able to pass a background check Must be able to pass a drug screen and breath alcohol test (if applicable) Must complete employee health meeting Education/Licensure/Certification: High School Diploma or equivalent preferred Unencumbered RN License Colorado or Compact State Current BLS Certification ACLS required within 1 year PALS required within 1 year Experience: Med/Surg experience preferred
