COVID VACCINATION (dose date on or after 10/3/23) NOW REQUIRED FOR THIS HEALTH SYSTEM.
Exemption forms are available in Trio/Downloadable Forms (Both forms REQUIRE Provider/Religious Leader Signatures) or please ask your delivery team for documents. IF APPROVED BY HEALTHSYSTEM TRAVELER WILL WEAR A MASK AT ALL TIMES WHILE ON SITE. To avoid delays in processing please include traveler covid vaccination card/or intent to fill out exemption form with profile submission. Thank you!
RATE WILL BE ADJUSTED FOR LOCAL TRAVELERS (10% reduction from posted rate). To avoid delays in processing please include perm address at upload/profile submission.
Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!!
NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). RTO for Memorial Day and 4th of July Holidays will not be approved!
NonBillable Orientation: 8hrs
OT Holiday multipliers: 1.25%

Requirement description :
- 2 years current Sterile Processing experience
- IAHCSMM
Profile Requirements:
- Work History
- Skills Checklist (pertinent to position) current to 1 year
- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications MUST be uploaded with profile (CSRT Cert Covid Vaccination proofs (dated on or after 10/3/23) or intent to file exemption form via link)
CERTIFICATION REQUIREMENTS :
- HSPA (IAHCSMM)
- BLS (AHA)
Weekend Requirements : As needed /Holidays as needed
