We are hiring a Diabetes Liaison Nurse for a Private Hospital in Dublin Ireland.

Responsibilities
We are hiring a Diabetes Liaison Nurse for a Private Hospital in Dublin Ireland.
We will provide exceptional patient care in an environment where quality respect caring and compassion are at the center of all we do.
Responsibilities
- To perform the necessary patient assessment planning delivery and evaluation of care for individuals with diabetes.
- Should take on the role of an expert and use their training and expertise to recognize and treat diabetic patients.
- To facilitate a smooth and safe transition for patients from outpatient treatment admission evaluation and hospital discharge by maintaining communication with all pertinent healthcare providers.
- To provide the best possible coordination and delivery of care the nursing team will work with you to assess plan deliver and evaluate outcomes.
- Maintain communication with and provide input to the diabetes team clinical facilitators and clinical nurse managers regarding any professional and educational resources that the clinical staff may need for diabetes education.
- Participate in any national strategies that are established as well as the auditing and data collection that occurs in the region.
Requirements
- A current NMBI General Nurse Registration is required.
- Officially recognised postregistration training pertinent to the field of advanced practice at the eighth level like a Graduate Diploma in Diabetes Nursing or an undertaking within a predetermined framework.
- Experience working in an acute setting for at least five years after registration.
- Possess two years of experience working in a clinically relevant environment.
- Candidates must be capable of teaching and have critical thinking problemsolving and leadership abilities.
- Must demonstrate multitasking ability.
