Retrieval and review of documentation in medical records from various client EMR systems.
Documentation audit & analysis for completeness and should follow the coding compliance guidelines
Auditing for relevant appropriate and accurate ICD 10-CM diagnosis codes HCC & HEDIS codes as needed adhering to the general payer and client specific Guidelines
Eye for detail finding errors and its trend in documentation .
