Position Summary

The Pharmacist provides comprehensive pharmacy services to nephrology patients with cross-coverage to other areas as required.

Duties

As a member of the patient care team the principal responsibilities include:

  • conducting best possible medication history (BPMH) interviews performing medication reconciliation reviewing and dispensing all doctors medication orders for appropriateness;
  • identifying resolving and preventing drug therapy problems with the patient and health care team;
  • monitoring drug therapy for effectiveness adherence to evidence-based medicine/guidelines; conducting medication reconciliation
  • following-up on medication incidents and promoting safe medication practices on the unit;
  • responding to drug information questions; providing patient education; facilitating hospital discharge;
  • and providing direct and/or functional supervision of technician staff. 
  • In addition the pharmacist will facilitate and support clinical teaching and research activities within the department while exemplifying UHNs values of caring excellence teamwork innovation integrity leadership and respect.

Qualifications :

  • Possess a Bachelors of Science Degree in Pharmacy Doctor of Pharmacy degree or equivalent required
  • Certificate of competency from the Pharmacy Examination Board of Canada required
  • Active registration with the Ontario College of Pharmacists (Part A) required
  • Clinic practice experience with patient-centered care experience preferred
  • Hospital Pharmacy residency an asset
  • Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
  • Experience in an inpatient acute care setting is an asset
  • Excellent verbal and written communication skills
  • Excellent customer service and interpersonal skills
  • Excellent organizational and time management skills
