Job Summary

Tasks include:

  • Support and care for clients with empathy and expertise to improve their personal health and wellbeing goals.
  • Undertake coordination of supports and wellbeing services.
  • Provide initial early intervention Triage with clients supporting both physical and mental health services.
  • Undertaking of relevant case coordination activity including liaison with all stakeholders; provision of email updates; and creation of appropriate documentation such as a treatment plans or other documentation as required.

Qualifications :

Requirements/Qualifications

  • Experienced Nurse with full registration. 
  • Hold a current and valid Working With Children Check (WWCC)
  • Evidence of completion of First Aid (level 2) or in circumstances where staff have obtained Basic Life Support training within a hospital setting
  • Mental Health support service experience highly desirable.
  • Caring and empathic nature.
  • Experience care coordination or occupational rehabilitation.
  • Well-developed written and verbal communication skills.
  • Good IT and time management skills.
  • Strong customer service focus.
