About the role and where you will be working:

The role focus is the delivery of safe client centred and efficient services that produce the best quality service outcomes for our clients overall health & wellbeing. This will be achieved through collaboration with Home Care Operations division to support develop and grow Home Care Services

Your responsibilities will includebut not limited to:
  • Grow Home Care services as measured in number of Home Care clients and service hours delivered.

  • Foster and actively contribute to a culture of continual improvement and make a lasting positive impact on the organisation its people and the community we serve.

  • Support Senior Management with the planning management and delivery of strategic outcomes for SCCWAs Strategic Plan and work collaboratively with employees to achieve those outcomes.

  • To provide leadership and direction to ensure all clients high-quality care that meet or exceed the requirements of the Aged Care Standards.

About you:
In order to be successful in this role you will need to have at least 5 years experience in a similar role. You will also need a strong commitment to person centred care and able to work both independently and within a team. Most importantly youll have a genuine desire to make a positive difference in our residents lives.
