This is ahybrid locally based role combining working from home with being out in theNorthern Sydneycommunitywith a particular focus onHornsbyRydeDenistone Macquarie ParkMarsfield the autonomy to manage your region and build relationships whileremainingconnected to and supported by the broader FOCUS Connect team.

About the Role

As ourCare Partner Northern Sydneyyoullcombine care partnering with community engagement and growth.

Youllbuild relationships across the aged carehealthand community sectors to strengthen referral pathways and connect more older people with the support they work directly with clients and families to understand what matters to them provide meaningfulchoiceand help them make the best use of their eligible aged care funding.

Your responsibilities will include:

• Build strongcommunity and referral networksacross Northern Sydneydriven bypurposeto reacholder people in need andconnectthem with the right services andsupports.
• Identifyopportunities toincrease CHSP referrals grow Support at Home clientnumbersand strengthen FOCUS Connects local presence.
• Manage a portfolio ofSupport at Home and CHSP clients building trusted relationships with clientsfamiliesand representatives.
• Develop and review person-centred care plans that support client goalschoiceand effective use of available funding.
• Work collaboratively with our support teams to deliver responsive well-coordinated services whilemaintaininghigh standardsof quality and compliance.

About You

Werelooking for someone who genuinelylovesconnecting with people and being part of their local community.

Youllbeself-motivated organised and comfortable working independently with the initiative toget outinto the community buildrelationshipsand create equally comfortable connecting with local organisations as you are sitting with an older person and their family to understand what matters to them.

Youllbring:

• Experience in aged care home care community serviceshealthor a related sector witha track recordof achieving positive outcomes.
• Experience in care coordination casemanagementor a similar client-facing role.
• Strong networking and relationship-building skills with the confidence toestablishnew referral pathways.
• A results-focused self-motivated approachand the ability to work independently and manage competing priorities.
• A genuine connection to Northern Sydney and an understanding of the local community.
• Sound knowledge of person-centred care and contemporary aged care practice.
• Knowledge of Support at HomeCHSPand the Aged Care Quality Standards will be highly regarded.

A relevant tertiary qualification innursingallied health or a related discipline isdesirable but not essential.

Youllalsorequirerelevant employment screening and checks or be willing to obtain themand hold a current drivers licence with access to a comprehensively insured vehicle.
