What you will be doing in this role

We areseekinga compassionateadaptableand experiencedSocial Workerto join our multidisciplinary team in a casual capacity. This is an excellent opportunity for a skilled clinician who enjoys the variety and fast pace of a hospital environment and is passionate about supporting patients and their families throughthe challenges they face during hospitalisation and discharge.

By collaborating with relevant multi-disciplinary team members you will lead and coordinate the safe transition of patientsback with the care team about new admissions and upcomingdischargesandrecommend services to meet patients healthcare a key role in liaising with patient familiesandinitiateconferences and action plans.

This is a Casual positionto cover ad-hoc leave for the team. Shifts are Monday-Fridaybetween8amand4.30pm.

Whowerelooking for

• Youllhavea Social Work degree from a recognised university.
• Eligibility for membership with the Australian Association of Social Workers (AASW).
• Recent experience in discharge planning within an acute care setting.
• Strong communicationskills with the ability to engage professionally and compassionately with patients familiesvisitorsand colleagues.
• A collaborative and adaptable approach with the ability to work effectively both independently and as part of a multidisciplinary team.
• An empathetic organised and detail-oriented work style along with a commitment to upholding Adventist Healthcares values of excellence and integrity.
