Job Summary

Epworth Rehabilitation has an exciting full-time opportunity for a Grade 1 Social Worker looking to further develop their clinical skills and healthcare experience within a leading hospital setting.  The role is based at Epworth Camberwell and is a permanent full time position of 38 hours per week.

The successful applicant will provide excellence in patient care predominantly in the area of aged care. Clinical responsibilities will include undertaking comprehensive psychosocial assessments family work outpatient work facilitating group education sessions and effective discharge planning within a multidisciplinary team. The role will be based at Epworth Camberwell with the possibility of working across other interesting clinical areas at our other rehabilitation sites at Hawthorn and Richmond.

The successful applicant will be joining an energetic and cohesive clinical team that strives for excellence in patient care and outcomes.  The Social Work department offers a high level of clinical support and supervision as well as a strong commitment to professional development activities and quality projects.

In this role you will:

  • Undertake comprehensive psychosocial assessments and interventions
  • Facilitate family meetings complex discharge planning and coordinate community supports
  • Provide counselling and support to patients and families adjusting to illness injury or changing circumstances
  • Promote patient and family-inclusive practice throughout all stages of care
  • Advocate for patients while balancing organisational and clinical priorities
  • Work collaboratively with multidisciplinary teams to achieve the best possible patient outcomes
  • Contribute to the ongoing development of social work services internally and externally
  • Share knowledge clinical expertise and practice wisdom with colleagues
  • Deliver an exceptional patient-centred care experience

Highly regarded experience includes:

  • Navigating the aged care system and supports available
  • Residential aged care placement processes
  • Carer stress and family support
  • Guardianship and VCAT matters
  • NDIS applications planning and support coordination
  • Family Violence elder abuse homelessness
  • Knowledge of compensable systems ie. TAC and VWA

Position Description      Inherent Requirements


Qualifications :

To be successful you will have:

  • A Bachelor or Master of Social Work qualification
  • Eligibility for membership with the Australian Association of Social Workers (AASW)
  • A minimum of one year of relevant social work experience in a hospital rehabilitation or healthcare setting
  • Well-developed skills in psychosocial assessment intervention and discharge planning
  • Experience supporting patients and families with adjustment to illness counselling community liaison and care coordination in a subacute healthcare environment
  • Strong communication interpersonal and stakeholder engagement skills
  • A commitment to excellence in patient care and collaborative practice
  • Enthusiasm professionalism and a desire to contribute positively to Epworths culture and values
