About the Role:

Join Country Health Connect as a NDIS Coordinator and make a meaningful difference by helping people with disability achieve their goals and live more independently. Working within a supportive multidisciplinary team youll coordinate person-centred services build strong relationships with participants and their support networks and deliver high-quality tailored support that promotes choice independence and positive outcomes.

What Were Looking For:

Essential Criteria

• Strong communication and relationship-building skills with participants families carers and stakeholders.
• Ability to work independently and collaboratively using sound judgement and initiative to achieve outcomes.
• Strong organisational analytical and problem-solving skills with the ability to manage competing priorities.
• Experience working in community and multidisciplinary settings with proficiency in Microsoft Office applications.
• Knowledge of confidentiality principles and person-centred consumer-directed care.

Desired Criteria

• Certificate III or IV in Aged Care Community Services or Disability Services.
• Knowledge of the National Disability Insurance Scheme (NDIS) and financial reporting requirements.
• Positive adaptable attitude with a willingness to learn and embrace change.
