About the role
Monday to Friday position within the Transitional Aged Care Program (TACP) providing an aged care package of time limited services (8-12 weeks) to clients following discharge from a hospital setting and who have goals to manage at home with multidisciplinary input and support services
Deliver comprehensive nutrition services to TACP clients in the community to improve physical function reduce risk of hospital readmission and support maintenance of independence in activities of daily living
Responsible for clinical supervision of junior staff and students and actively participate in and lead quality improvement and service development initiatives
Provides case management duties as required including coordination of care for clients over the duration of their Transition Care Program (TACP)
Provides leadership within the multidisciplinary TACP team supporting effective service delivery promoting collaborative practice and contributing to positive client outcomes across the program
About you
Current unrestricted NSW Drivers License (Class C or above)
Demonstrated extensive post graduate experience as a Dietitian and a degree in Nutrition and Dietetics acceptable to the Dietitians Australia preferably with APD status
Extensive clinical experience in nutrition assessment treatment and education within aged care/community setting with evidence of sound clinical judgment in solving problems and managing complex clinical cases
Demonstrated initiative and with experience in leading quality improvement and service improvement projects
Demonstrated ability to work autonomously with minimal direct supervision with demonstrated experience in providing clinical supervision to clinical staff/ students
