Senior Case Manager

About the role:

The Senior Case Manager is responsible for the provision of case management and support services to young people living in residential homes and will also participate in providing specialist services to young people aged 12 to 19 years old in MacKillops OoHC program and after leaving care for a maximum of 12 months.

The Senior Case Manager will provide outreach services to MacKillops OOHC Clients after leaving care develop and implement relevant programs according to client needs undertake all outlined duties expected in case management including but not limited to: end to end case management of clients accurate and timely case notes in line with MacKillop expectations participation in associated meetings and provision of individual support and guidance to Young People develop and support leaving care arrangements provide expert advice to the case management team regarding complex cases provide reports on progress of programs including recommendations and under general and limited direction undertake a variety of tasks as delegated by the Case Management Coordinator

This is a Full-Time opportunity based in Forest Hill VIC within our Out of Home Care Program. The position reports to the Case Management Coordinator.

Key Selection Criteria:

To be successful in this role the incumbent will have:

• Tertiary qualifications in a relevant field with substantial relevant experience
• Demonstrated experiences in residential care child protection child welfare practice or a related field.
• Demonstrated professional casework skills.
• Demonstrated assessment skills and the ability to use these skills in a range of setting
• Sound professional knowledge and theory base including a solid understanding of trauma informed practice.
