Your opportunity
Support people in recovery to build independence wellbeing and community connection.
This role will provide person-centred support to individuals accessing residential mental health services working within a recovery-oriented framework to empower clients to achieve their goals and strengthen their independence. You will collaborate with internal teams and external services to deliver high-quality support that promotes recovery community integration and wellbeing. The role operates across a 24/7 roster including days nights weekends and public holidays.
This is an opportunity to make a meaningful difference by supporting people through daily living activities case management community engagement and transition planning in a safe and supportive environment.
This is a max term role with contract ending on 30th June 2027.
Your key responsibilities will be to:
- Deliver person-centred and culturally responsive case management support
- Conduct intake assessment registrations and support the clients with activities of daily living and recovery goals
- Develop holistic and strengths-based individualised case plans
- Deliver practical group-based program and training sessions and facilitate referrals to specialist services
- Maintain accurate client records reports and case management documentations
- Participate in team meetings and training activities
- Contribute to effective operation and continuous improvement of the services
- Build positive and sustainable relationships with clients colleagues and internal and external stakeholders
Requirements for success
- Strong understanding of mental health recovery-orientated practices
- Degree in social work or related discipline
- A positive person-centred approach with strong communication and interpersonal skills
- Strong organisational and administrative skills
- Resilience and ability to work with complex environments
Before starting work with us you will need to undertake a national police history check qualifications and referee checks.
