About the role

As a member of the Calvary Home Care team you will deliver quality services to meet the needs of our clients and contribute to a positive experience whilst at Calvary Community Care. Assist in planning assessing and evaluating our clients needs with the help of our multidisciplinary team to promote positive health outcomes. The Registered Nurse provides clinical support to our clients and collaborates with our support workers to deliver exceptional care to our clients in the community. Role will include attending client assessments and clinical supports as well as training and support of staff and hospital rounding.

Key Responsibilities

  • Work with clients and families to identify and meet care goals
  • develop and maintain comprehensive assessments to ensure client safety and care needs met
  • coordinate provision of nursing care including assessments wound management and liaise with Case Managers and multidisciplinary health care team

About You

  • AHPRA Registration and current First Aid Certificate
  • Recent nursing/clinical experience in the last 5 years or evidence of a refresher course
  • Effective communication interpersonal and organisational skills
  • Initiative and the desire to continually improve client care
  • Capability to prioritise tasks and achieve goals within set timeframes
  • Candidates with an understanding of the community nursing sector - Home Care Packages Department of Veteran Affairs and Commonwealth Home Support Program will be considered favourably
  • A desire to join a team committed to the care of others in a mission based organisation
