Job Summary

As a Registered Nurse - Home Care you will promote and deliver a holistic client-centered approach to care focusing on client needs and rights in line with Consumer Directed Care principles. You will contribute to assessments prevention early detection referrals and health promotion activities providing quality care that aligns with current nursing care plans. You will deliver comprehensive safe and effective evidence-based care to achieve targeted health outcomes while ensuring full compliance with relevant regulations and organisational policies.
Responsibilities include:
- Promotes and delivers a holistic approach to care focusing on customer needs and rights in alignment with Consumer Directed Care and delivers the assessment prevention early detection referral and funding activities.
- Delivers comprehensive safe and effective evidence-based care to achieve optimal health outcomes for clients.
- Drives compliance with common law regulatory and state legislation and organisational policies and procedures relevant to the registered nursing practice.
- Drives the formulation review and case conference of care plans as needed ensures they accurately reflect customer needs and support Aged Care Funding Instrument (ACFI) funding.
To be successful in this role our preferred candidate will have:
- Bachelors Degree required; Nursing Healthcare is preferred. Any other related discipline or commensurate work experience considered.
- Minimal experience as a Registered Nurse in a hospital clinical or residential setting with expertise in patient care and medical procedures is preferred.
- Registered Nurse with Australian Health Practitioner Regulation Agency (AHRPA) Certification is prescribed.
- Strong knowledge of Clinical Assessment Medication Administration & Monitoring Person-Centered Practice and Quality Care Service Management skills.
- Excellent communication adaptive thinking planning and prioritisation and cross-functional team working skills.
