As part of the Bringing Care Home strategy we are scaling connected digitally enabled chronic care models nationally. These models combine technologydataandhumancentreddesign to make care more accessible moreequitableand more sustainablefor people living with ongoing health needs.

Werenow seeking a Product Analyst ChronicCareto help design optimise and scale these models of care.

The opportunity

This role sits at the heart ofcrossfunctionaldelivery.As Product Analyst ChronicCare you will work across clinicaloperationaland digital teams to turn complex chronic care needs into clear requirements efficientworkflowsand intuitive digital solutions. Your work will directly enable better experiences for patientscarersand cliniciansand support the safe scalable growth of Care at Home services.

You do not need a clinical background. You do need curiosity strong analyticalthinkingand a passion for solving meaningful problems at scale.

Whatyoulldo

  • Translate complex needs into solutions
    Work with clinicians and service teams to translate chronic care needs into functional requirements user stories and process designs that supportpersoncentredcare.

  • Optimise products and processes
    Analyse and improveendtoendworkflows across virtual andhomebasedservices strengthening interoperabilityefficiencyand user experience.

  • Use data to drive improvement
    Monitor product performance patient experience equityimpactsand system usage toidentifyopportunities for optimisation and inform business cases.

  • Partner across disciplines
    Act as a bridge between clinicaltechnicaland business stakeholders to align priorities and deliver scalablehighqualitysolutions.

  • Support innovation and quality
    Contribute to continuous improvement by integrating analytics emergingtechnologyand automationwhile ensuring compliance with clinical governance safetyprivacyand regulatory standards.

About you

You are a collaborativedatadrivenproblem solver who enjoys working in complex environments and translating insight into action.

You bring:

  • Experience working as a business analyst product analyst or similar role in digitalplatformor service environments

  • Strong ability to turn user consumer or customer insights into clear requirements and practical solutions

  • Systems thinking with sound business and analytical capability

  • Confidence working across diverse stakeholders and influencing without authority

  • Ability to interpret complex data research or policy and translate it into operational improvement

Experience in digital health virtual care or chronic care services is highly regardedbut not essential.
