Program

Caulfield Community Health Service (CCHS) provides a range of clinical and health promotion services for people of all ages who live work or study within the local community. These multi-disciplinary services are provided in the Centre community home or via telehealth. CCHS operates from the Caulfield Hospital site at Alfred Care Group.

Position Summary

The podiatrist will work within the multi-disciplinary Adult Health Team to provide high-quality clinical services for people in the local community living at home. The job will involve comprehensive podiatry assessment and interventions which provided within a clients home the CCHS clinic and a half day in the Caulfield Chronic Wound Clinic co-located at the Caulfield Hospital site at Alfred Care Group.

The podiatrist will work collaboratively within the team and with external agencies to ensure a holistic approach to client care. The position will be structured with an 80% clinical and 20% non-clinical workload and will receive support guidance and direction from the CCHS Grade 3 Podiatrist.

Key Duties And Responsibilities

  • Providing high-quality podiatry services for the local community. This may include initial and annual foot assessment routine and pathological nail care safe and effective sharps debridement of callus wound management nail surgery biomechanical assessment prescription and/or manufacture of orthotics
  • Provide podiatry interventions at the centre in the clients home or local community
  • Ensure podiatry clinical practice is evidence based works from the principles of best practice and is tailored to meet the individual needs of clients
  • Develop and implement person centred care plans
  • Work within a multi-disciplinary team to ensure a holistic approach to client support
  • Maintain current knowledge of service specific guidelines and performance targets and incorporate these into work practice
  • Plan and manage own workload to ensure adequate time for the achievement of primary objectives of the position
  • Supervise undergraduate podiatry students and may supervise the Grade 1 Podiatrist
  • Liaise with relevant agencies to coordinate facilitate and communicate client treatment and outcomes
  • Maintain current knowledge of service specific guidelines and performance targets and incorporating these into work practice
  • Enter accurate service provision data for reporting as required
  • Document and collect service fees and other monies as required
  • Demonstrate an understanding of pathways within the health care system in order to help facilitate appropriate care and advocate for clients with complex care needs (e.g. My Aged Care NDIS Commonwealth Home Support Program Home Care Packages)
  • Contribute to the development of relevant policies guidelines and work instructions to promote excellence in the provision of services
  • Plan and manage own workload to ensure adequate time for the achievement of primary objectives of the position
  • Identify research and quality improvement priorities
  • Participate in clinical with Grade 3 Podiatrist and operational supervision with Team Leader
  • Demonstrate an openness to feedback and a commitment to ongoing development of clinical capabilities
  • Engage in performance management planning and review in line with organisational policies
  • Identify needs and opportunities for self-learning and professional development including in-service internal and external courses seminars conferences etc.
  • Meet individual and team KPIs and targets in order to meet funding requirements

Qualifications/Experience Required

Essential

  • Tertiary qualification in podiatry
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Demonstrated podiatry clinical experience relevant to the community health sector
  • Experience in working as part of a multidisciplinary team to provide comprehensive and holistic services
  • Capacity to work independently and autonomously and yet facilitate a collaborative approach
  • Understanding of and alignment with the Bayside Health capabilities
  • Demonstrated commitment to continuing professional development
  • Capacity to understand and adhere to organisational policies and guidelines
  • Excellent communication skills (verbal and written) collaboration and problem-solving skills
  • Well-developed computer skills
  • Current Victorian drivers licence

Desirable

  • Membership with the Australian Podiatry Association
  • Membership with Wounds Australia
  • Experience in working within the community health sector
  • Experience in supervising podiatry students
