Grade 2 Occupational Therapist - Older Persons Care Hub (OPCH)

The Department

The Older Persons Care Hub (OPCH) is a new grant-funded pilot program within the HAC portfolio designed to enhance integrated care for older adults through multidisciplinary home-based assessment and intervention.

The OPCH aims to reduce avoidable hospital presentations and admissions by supporting older people to safely remain in their homes. The OPCH will be based alongside the Mobile Assessment and Treatment Service (MATS) and will work closely with GPs and EDs for referrals in the first instance. This initiative will add to the growing suite of home-based services provide by the HAC Program ensuring coordinated transitions of care and continuity of support. The pilot will run for an initial 1218-month period with patient care commencing from January 2026.

The Role

We are seeking an enthusiastic and passionate Grade 2 Occupational Therapist to join the team in the new Older Persons Care Hub (OPCH). The OPCH Occupational Therapy role is part of a new pilot program designed to support Alfred Healths home-based and virtual models of care reducing unnecessary hospital presentations and admissions for older people. The service provides timely multidisciplinary care to community-dwelling older adults within the Alfred Health catchment targeting those with immediate cognitive concerns functional and mobility needs. The OPCH OT will conduct comprehensive home-based assessments deliver short-term interventions and ensure safe home environments through provision of equipment education and advice. They will also facilitate referrals to community services to support ongoing care needs. The OT will collaborate with physiotherapy nursing medical pharmacy and social work colleagues to optimise patient outcomes and maintain independence at home. They will be well supported by OPCH Team Lead and the OT department including regular supervision and access to professional development opportunities.

Qualifications and Experience Required

  • A minimum of 3 years of clinical experience in occupational therapy
  • Registration with the Occupational Therapy Board of Australia and eligibility for OTA membership
  • Clinical experience in subacute occupational therapy and knowledge of client-centred occupational service delivery for older patientsparticularly in the areas of dementia and delirium.
  • Demonstrated ability to evaluate and modify service delivery in accordance with evidence-based practice
  • Experience working within an interdisciplinary team to provide high-quality rehabilitation
