Join our Community Aged Care team as an Occupational Therapist and empower older people to live safely confidently and independently in the homes and communities they love.

What youll do

As an Occupational Therapist within our Community Aged Care team youll help older people maintain their independence safety and confidence while continuing to live in their own homes.

Working with customers across our TCP CHSP & Support at Home services youll enjoy a varied caseload and the opportunity to support each customer throughout their therapeutic journey - from assessment and goal setting through to intervention review and practical outcomes.

Our consistent referral pipeline allows us to geographically zone caseloads helping to limit travel and giving you more time to focus on meaningful clinical work. Youll also join us during a period of growth and innovation with opportunities to contribute your ideas as we strengthen electronic customer records streamline workflows and develop more effective ways of delivering care.

Youll be trusted to make a valuable impact by:

  • Delivering evidence-based Occupational Therapy assessments and interventions that support independence in daily living at home
  • Supporting customers to achieve meaningful reablement-focused goals aligned to their individual needs and preferences
  • Supporting customers across different stages of their therapeutic journey and reviewing progress against agreed goals
  • Recommending equipment home modifications and strategies that enhance safety and participation
  • Collaborating with families carers and multidisciplinary team members to strengthen outcomes and continuity of care
What youll bring

To thrive in this role you will have:

  • Tertiary qualification in Occupational Therapy and current AHPRA registration
  • Strong understanding of ageing reablement principles and person-centred practice
  • Excellent communication skills and ability to build trusted relationships with customers and stakeholders
  • Current drivers licence and willingness to travel across the community

We would also love you to have:

  • Experience working in community aged care or home-based service delivery
  • Experience recommending assistive technology equipment or minor home modifications
  • Interest in contributing to service improvement and innovation within Community Aged Care Allied Health services
