About the Role
Lumiere Foundation is seeking an experiencedNDIS Support Coordinatorto work withICCareparticipantsin greater Sydney.
In this role you will provide varying levels of Support Coordination and Transition of Supportsassistingparticipants to implement their NDIS plans connect with services and achieve their goals while exercising choice and control.
In This Role You Will
- Apply and adhere to NDIS and Child Safe standards and regulations
- Efficiently coordinate supports for participants withinallocatedfunding levels
- Liaise and collaborate with participants families service providers and other stakeholders to achieve participant goals
- Develop andmaintaineffective service linkages and community connections
- Empower participants to exercise choice and control through formal informal mainstream and community-based supports
- Identifyopportunities for innovative and person-centred service delivery
- Maintainaccuraterecords case notes files and reports in line with NDIS requirements
- Ensure compliance with NDIS Quality and Safeguards standards and legislative requirements
WhatYoullBring
- Tertiary qualification in Social Sciences Health Community Services or a related field or a Diploma in Disability Services
- Minimum2 years experiencein NDIS Support Coordination
- Strong understanding of theNDIS Quality and Safeguards Commission Practice Standards
- Experience working within the disability and community services sectors
- Proficiencyin Microsoft Office Suite (Outlook Word Excel)
- Experience using client management systems such asMYPLumaryShiftCare or similar
- Excellent verbal and written communication skills
- Strong report writing and documentation skills
- Ability to speakArabicis highly regarded but not essential
Essential Requirements
- Full Australian working rights
- Current National Police Check
- Current First Aid Certificate
- NSW WorkingWithChildren Check (WWCC)
- NDIS Worker Screening Check
- Unrestricted NSW Driver Licence
- Access to a reliable insured and roadworthy vehicle
