Amity HealthareseekingadedicatedIntegrated Chronic Disease (ICDC) Care Coordinator based in Moora WA to join our ICDC Team.

In this role youll work closely with General Practitioners and other Health Professionals to improve access to primary care strengthen continuity of care and support improved health outcomes. Youllprovide proactive coordination across the care continuum support access to supplementary services and contribute to building the cultural capability of mainstream primary health providers.

Program Overview:

Amity Health delivers the Integrated Chronic Disease Care (ICDC) program on behalf of the WA Primary Health Alliance across the Great Southern Wheatbelt and South Coastal Goldfields regions. The program supports adults aged 18 years and over who are living with chronic conditions including cardiovascular disease diabetes respiratory disease and morbid obesity and who require additional care coordination to improve health outcomes.

ICDC provides structuredGP-referredcare coordination to support integrated management of chronic disease. Care Coordinators work collaboratively with General Practitioners and allied health providers to assess needs facilitate appropriate referrals support implementation of GP Management Plans and enhance clients capacity to self-managetheir condition. Services are delivered viain-personand telehealth appointments to ensure access across regionalcommunities.

About the Role.

This is a meaningful opportunity to make a genuine difference. As a compassionate clinician you will:

• Provide culturally safe person-centred care coordination to our clients enrolled in the ICDC program supporting improved management of chronic health conditions.
• Support clients to engage with their GP and relevant health services assisting them to understand and follow agreed care plans referrals and appointments.
• Identify and address cultural systemic and practical barriers to accessing care and advocate on behalf of clients within mainstream primary care settings.
• Build and maintain strong working relationships with GPs allied health providers and other key stakeholders and participate in coordinated service planning.
• Administer ICDC Supplementary Services funding in line with program guidelines and organisational policy ensuring appropriate equitable and well-documented use of funds.
• Maintain accurate client records and program data meet reporting and contractual requirements and contribute positively to team meetings supervision and service planning

This role is part-time 3 days per week based in Moora WA.

About You

You are a compassionate clinician who is committed to walking alongside our clients supporting them to access the right community supports and achieve improved health and wellbeing outcomes.

To be successful in this role you must meet the following eligibility and qualification requirements:

• Hold a tertiary qualification in a relevant health discipline (e.g. nursing allied health social work or similar) that meets the requirements for classification as a Health Professional Level 2
• Demonstrated experience providing care coordination case support or service navigation in a health or community setting
• Ability to work collaboratively with GPs and multidisciplinary health professionals
• Strong organisational time management and record keeping skills
• Effective communication skills both written and verbal
• Ability to work autonomously with defined policies and procedures
