As a Support Worker youll play a vital role in enhancing the independence and well-being of our clients. Your responsibilities will include:

• Providing Personalised Support: Deliver high-quality individualized assistance that empowers older adults to live fulfilling lives.
• Creating Connections: Offer companionship and emotional support through engaging conversations and meaningful interactions.
• Facilitating Independence: Help clients with personal care and transportation to appointments ensuring they stay connected and active in their communities.
• Implementing Care Plans: Follow and execute individual service healthcare and positive behavior support plans tailoring your approach to each clients unique needs.

Who we are looking for
If you have experience working as a Support Worker are qualified with a Minimum Cert III in Aged Care Community Support or related field and enjoy variety in your day you might be exactly the person were looking for!

• Minimum Cert III Qualification in Aged Care or Individual Support (must have completed aged care components).
• Experience working with older adults.
• The ability to follow and implement individual service plans healthcare and positive behaviour support plans.
• A desire to use your skills and experience creating fun and interesting activities that support older adults to engage more consistently with their community and peers.
• A valid drivers licence.

Qualifications :

• Minimum Cert III Qualification in Aged Care or Individual Support (must have completed aged care components).
