About The Role

Want to be part of a team of professionals who take pride in providing the highest levels of participant care The Fortify Health Care Services team is supportive approachable and client-centred and also provides excellent staff training and development opportunities through our training academy.

We provide care and support to participants with complex health needs including mental health challenging behaviours and high-intensity support needs.

The Enrolled Nurse is responsible for the delivery of safe and effective nursing care under the direction of the Registered Nurse (RN).

This is not agency work. Youll work consistently with the same participants supporting them in the comfort of their own homes. That means more meaningful relationships greater continuity of care and the opportunity to truly make a difference.

Pay Rate:

Enrolled Nurses $44.03 super penalties (inc. cas. loading)

Duties & Responsibilities:

  • Adhere to positive behaviour support plans & implementing documented strategies to best support positive outcomes for the participant.
  • Assist with high intensity support needs: diabetes management and sub-cut injections.
  • Deescalate and provide mental health support and manage & provide a safe & calm environment.
  • Attend to personal cares catheter flushes medication administration and relevant high-intensity support requirements.
  • Support participants with daily supports including light domestic tasks transport meal preparation companionship.
  • Approach complex situations with calm compassion and professionalismyour presence can be a powerful source of reassurance.
  • Use encouragement and motivation to support participants to independence and other personal goals.
  • Work closely with your fellow team members to create a smooth stable and positive service environment as a team player. If a colleague needs help lend a hand. Weve got each others backs.
  • Complete required documentation including shift notes incident reports and data collections via in-house support charts and contribute to the development modification & evaluation of Participant Care Plans.
  • Build strong rapport with participants and stakeholders.
  • Attend to medical emergencies & provide first aid as required.
  • Have a positive can-do attitude with the ability to focus on a holistic approach to support encompassing a wellbeing and clinical lens within your support framework.

What Youll Bring

  • Current AHPRA registration as an Enrolled Nurse with no restrictions (ENs)
  • Relevant nursing qualifications for EN roles
  • Work a rotating roster with a willingness to work night duty (if required) including inactive and active sleepovers
  • Lived or worked experience supporting participants with complex health mental health and positive behaviour support plan implementation
  • A minimum of 6-12 months of experience supporting participants with complex health and mental health needs
  • Excellent technological communication and time management skills
  • Valid First Aid and CPR certificate at the time of application
  • Relevant compliance checks including valid NDIS Worker Screening Check & Blue Card at the time of application
  • Valid drivers license and have own vehicle
