How Youll Make an Impact As a Disability Support Worker with One Doorway you will join a dedicated team of carers who provide person-centred support to people with disabilities in their own homes. A day in the life as a Disability Support Worker with One Doorway may include: Supporting participants to work towards their goals in alignment with their NDIS plan with a strong focus on safety dignity and individual outcomes Providing in-home support with health requirements personal care and daily living tasks in line with participant needs and Care Team support plans Delivering person-centred care and support based on individual needs and support plans. Including the delivery of activities that build capacity and develop skills across physical social emotional and intellectual domains Completing accurate progress notes and documentation after each shift. Working respectfully alongside families health professionals and internal care teams to achieve great outcomes while developing and maintaining positive relationships with all stakeholders. Where required provide: Behavioural and Mental Health Support Assistance with medication administration Expertise working with a range of Disabilities As a Support Worker you will ideally have experience within the disability and/or healthcare sectors. You will also demonstrate the following: A strong commitment to the rights of people with disabilities to live safe dignified and meaningful lives A patient compassionate flexible understanding and supportive nature. With excellent listening skills and strong attention to detail with a genuine desire to make a positive difference in peoples lives The ability to take responsibility use initiative and work effectively as part of a team Previous experience in a similar role particularly working with individuals requiring trauma-based care is highly desirable. Ability to work independently as well as part of a multidisciplinary team Desirable however not essential: Certificate III in Individual Support or equivalent. Valid drivers licence with access to a comprehensively insured vehicle.
