About theRole

As a Support Worker you will provide individualized person-centred support to adults young people and children with disability and complex support needs.

You will work alongside participants families carers and community networks to support individuals to achieve their goals while promoting dignity independence choice and inclusion.


Key Responsibilities
  • Support participants with daily living activities and personal care
  • Assist with household and domestic tasks
  • Encourage independence and skill development
  • Support community participation and social inclusion
  • Implement individual support plans and goals
  • Follow Positive Behaviour Support Plans (BSPs)
  • Administer medication in accordance with policy and individual health plans
  • Accurately complete progress notes and documentation
  • Build positive relationships with participants families carers and stakeholders
  • Ensure participant safety well-being and safeguarding requirements are met
  • Promote participant rights dignity and self-determination
