Job Summary
Are you passionate about helping older Australians remain independent in their own homes Healthcare Australia is seeking a people-focused Home Care Coordinator to support clients across the Tamworth area.
In this rewarding and varied role youll build strong relationships with clients and their families coordinate services that meet individual needs and act as a trusted point of contact throughout their Home Care journey. Youll spend time meeting with clients conducting assessments developing care plans and working closely with internal teams to ensure exceptional service delivery.
This role would suit someone with experience in Home Care aged care community services healthcare support coordination case management or client-facing customer service who enjoys building relationships and making a genuine difference in peoples lives.
You may have experience in Home Care aged care disability community services healthcare customer service care coordination or case management. Most importantly youll be someone who enjoys working with people building trust and delivering outstanding customer experiences.

You are organised proactive and confident managing multiple priorities while maintaining accurate documentation and compliance requirements. Youll have excellent communication skills a customer-first mindset and the ability to build strong relationships with clients families referrers and colleagues.
Knowledge of Home Care or Support at Home services will be highly regarded although we are also interested in speaking with candidates from broader healthcare and community services backgrounds.
