Community Services Manager

Lead the next chapter of community care at Village Baxter. We are seeking an experienced senior leader to oversee a diverse community-care portfolio and build a strong accountable and sustainable service model for older people living at home.

Salary $145K plus super and access to salary packaging

Reporting to the Director of Clinical Services the Community Services Manager will lead the operational performance of Village Baxters community care services including Support at Home Commonwealth Home Support Programme (CHSP) Veterans Home Care and associated community nursing/support services.

This genuine senior leadership role is full time on-site and combines people leadership service quality compliance financial management and sustainable growth all while ensuring clients receive safe responsive and person-centred support.

• End-to-end operational delivery across Support at Home CHSP and Veterans Home Care.
• A multidisciplinary community-care workforce with clear accountability coaching and performance expectations.
• Quality safeguarding incidents complaints risk audit readiness and continuous improvement.
• Budgets funding claiming billing service utilisation workforce efficiency associated compliance obligations and sustainable growth.
• Client engagement intake assessment care planning review and service coordination systems.
• Strong relationships with clients families referrers My Aged Care DVA health professionals and community partners.
• Clear monthly KPI/KRI reporting and early escalation of material risks.

• You have current AHPRA registration.
• You are an experienced community aged-care or health-services leader who can move comfortably between strategy and operational detail.
• You understand Support at Home and CHSP; DVA/Veterans Home Care knowledge would be highly regarded.
• Your servant leader style is based on support that builds capable teams holds people accountable and addresses performance issues constructively and promptly.
• You have strong financial and operational judgement and understand how quality workforce and commercial performance connect.
• You are calm organised and solutions-focused with strong professional boundaries and the confidence to make decisions within your authority.
• You bring relevant tertiary qualifications in nursing allied health community services health/service management business or a related field.

Village Baxter is an independent long-standing not-for-profit Charity supporting older people across retirement living residential aged care and community services. We are entering a new phase of renewal with a strong focus on connection accountability respect and excellence. Our Support at Home service program is focused on clients within our 76-acre retirement village and the immediate surrounding local Frankston / Baxter area. We know who we are and what we do well and want to focus on our strengths and sustainability.
