About the Program

The Permanency Support Program is a key NSW child protection initiative designed to provide safe stable and nurturing long-term outcomes for children and young people who cannot live safely with their families. Veritas House partners in collaboration with the Department of Communities and Justice(DCJ) focusing on achieving timely permanency through restoration guardianship open adoption or long-term care. Our team work closely with children families carers and communities to support healing strengthen relationships and promote lifelong connections through tailored case planning therapeutic supports and carer development the PSP aims to improve stability wellbeing and life outcomes for children and young people.

About the role

As a Case Worker in our Permanency Support Program youll play a vital role in the statutory out-of-home care (OOHC) system working with children and young people who have experienced trauma loss and instability. Youll provide comprehensive case management to help them feel safe valued and connected while working towards permanency and long-term stability.

This is a challenging yet deeply rewarding role where no two days are the same. Youll collaborate with DCJ carers birth families and specialist services to deliver the best-practice care planning principles that meets legal emotional and developmental needs. Youll also empower young people in Supported Independent Living to build the skills and confidence needed to transition successfully into adulthood.


Specific responsibilities of this role include:

  • Provide high-quality case management to children and young people in Foster Care and Supported Independent Living.
  • Develop and implement case plans that promote safety stability and positive life outcomes.
  • Support young people to build independent living skills manage tenancies and access education training and employment opportunities.
  • Build strong respectful relationships with children young people Foster Carers and families.
  • Collaborate with service providers and government agencies to ensure the best outcomes for each child or young person.
  • Maintain accurate documentation and ensure compliance with Permanency Support Program and NSW Child Protection standards.

Essential Criteria:

  1. A relevant qualification in Community Services Social Work Youth Work Case Management or a related field (minimum Diploma level).
  2. Experience in child protection out-of-home care or youth services.
  3. Strong understanding of the Children and Young Persons (Care and Protection) Act 1998 and NSW Child Safe Standards.
  4. Excellent case planning documentation and time management skills.
  5. The ability to engage effectively with children young people carers and families.
