As a full-time Case Manager based in Baulkham Hills with Lifestyle Solutions youll play a vital role in supporting children young people families and carers through strength-based person-centred case management. Youll work collaboratively to deliver meaningful outcomes by developing tailored support plans coordinating services and advocating for the young people in your care.

What Youll Bring

• Demonstrated experience in case planning for children and young people preferably within Foster Care or Out-of-Home Care
• Knowledge of the Children and Young Persons (Care and Protection) Act 1998
• Experience supporting children and young people who have experienced trauma disability or complex behaviours
• Strong analytical assessment and problem-solving skills
• Excellent written communication and record-keeping abilities
• Ability to work flexibly and collaboratively with a range of stakeholders

What Youll Be Doing

• Manage a caseload of children and young people working closely with their families and carers to achieve positive outcomes
• Complete comprehensive assessments and develop implement and review individual case plans
• Coordinate intervention strategies including risk assessments referrals and program applications
• Prepare reports legal documentation and support child protection-related court processes where required
• Facilitate access to services and monitor their effectiveness in meeting individual needs
• Advocate for children and young people by ensuring their voices goals and wellbeing remain at the centre of care planning
