Case Manager – Intake and Assessment
Job Summary
Hutt St Centres vision is an end to homelessness. Our mission is to advocate with and empower people at risk of or experiencing homelessness supporting them to rebuild their lives on their journey to homefulness.
You will be joining a dedicated and passionate team who works hard to do their part in ending homelessness in South Australia.
How you will make a difference
As an Intake and Assessment Worker you will play a vital role as the first point of contact for people experiencing or at risk of homelessness. You will provide a welcoming and responsive entry into the homelessness service system delivering intake triage assessment and referrals both in person and over the phone.
Working collaboratively with the Toward Home Alliance you will use assessment tools to identify clients needs develop person-centred case plans and referral pathways and connect clients with the most appropriate housing and support services.
This position will be offered on a part-time (0.8 FTE) contract basis until 30 June 2027.
About you
You are a compassionate organised and client-focused professional with experience in assessment case management or community services. You have excellent communication and interpersonal skills manage competing priorities and work effectively both independently and as part of a team.
An understanding of homelessness trauma-informed practice and working with people from diverse backgrounds will be highly regarded along with experience using client management systems and assessment tools.
You must have or be willing to obtain both a satisfactory National Police Clearance and Working with Children Check and hold an unrestricted drivers license.
