PositionPurpose

The CaseCoordination Practitioner SEWB provides culturally safe trauma-informed andperson-centred support to Aboriginal and Torres Strait Islander peoplepresenting with multiple and complex needs.

The role delivers intensive case coordinationincluding:

comprehensive SEWB and psychosocialassessment

development and implementation ofclient-led case plans

therapeutic and brief interventions

warm and supported referrals

aftercare planning

ongoing monitoring of progress riskand wellbeing

This role strengthens social and emotionalwellbeing by supporting individuals and families experiencing complexchallenges including mental health concerns AOD issues grief and lossfamily stressors identity and connection needs and the impacts ofintergenerational trauma.

Practice is guided by Aboriginal ways of knowingbeing and doing and by national frameworks including the National StrategicFramework for Aboriginal and Torres Strait Islander Peoples Mental Health andSEWB the Cultural Respect Framework and the National Drug Strategy

KeySelection Criteria

Demonstrated in-depth understanding ofAboriginal social and emotional wellbeing including the impacts ofintergenerational trauma cultural identity community dynamics systematicdisadvantage and culturally grounded healing frameworks.

Proven experience delivering intensivesupport to clients with multiple and complex needs using coordinatedculturally safe and trauma-informed practice approaches.

High-level capability to undertakecomprehensive SEWB and psychosocial assessments identify risks and developclient-led case plans that integrate cultural strengths. Family involvement andappropriate supports.

Demonstrated ability to deliver brief therapeuticinterventions including motivational interviewing and other evidence informedapproaches and to provide warm supported referrals to culturally safe andmainstream services.

Exceptional ability to build trust and therapeuticrapport with Aboriginal people families and community members particularlythose experiencing trauma grief AOD concerns or mental health challenges.

Strong professional integrityincluding the ability to maintain confidentiality uphold ethical boundariesand apply sound judgement in complex and sensitive situations.

High standard of documentation and caserecording including accurate timely and culturally respectful case notesassessments plans and referral records.

Competence and confidence usingelectronic client management systems and Microsoft Office applicationsincluding the ability to meet reporting and data quality requirements.

Understanding of governance qualityand safety frameworks relevant to primary health and community services(including Child Safe Standards Social Services Standards Victorianinformation sharing schemes and clinical governance principles) and theability to apply these in day-to-day practice.

Qualificationsand Registrations Requirement (Essential or Desirable).

Valid Victorian and unrestricted driverslicence ESSENTIAL

Demonstrated understanding of Aboriginal cultureand the health and wellbeing needs of Aboriginal people ESSENTIAL

CertificateIV qualification (or equivalent experience) in Alcohol and Other DrugsCounselling Community Services/Social Work Support Mental Health or YouthWork demonstrating capability in trauma-informed practice culturally safeclient engagement complex needs assessment brief therapeutic interventioncoordinated care risk identification and the application of evidence-informedSEWB frameworks within primary and community health environments ESSENTIAL

A satisfactory Working with ChildrenCheck and Fit2Work Police Check ESSENTIAL

A diploma-level qualification (or equivalentexperience) in Alcohol and Other Drugs Mental Health Counselling CommunityServices or a related behavioural or social science field demonstratingadvanced capability in trauma-informed practice therapeutic engagementcomplex case coordination culturally safe service delivery and theapplication of evidence-informed approaches within primary and community healthsettings HIGHLY DESIRABLE

A person of Aboriginal and or Torres Strait Islanderbackground HIGHLYDESIRABLE

Membership (or eligibility for membership) with arelevant professional association such as ACWA PACFA ACA NAATSIHWP AASWANZMHA or a recognised AOD or mental health professional network HIGHLY DESIRABLE
