Job Summary
Your opportunityAs a Care Partner Support Officer youll play a vital role in supporting the Care Partner team through administration coordination and client caseload as a primary point of contact you will provide exceptional customer service while supporting the delivery of care under the Wellness Model of make an impact byActing as a key contact for clients families and advocates Supporting Care Partners with coordination scheduling and administrative tasks Managing a client caseload while monitoring wellbeing and outcomes Maintaining accurate timely documentation across systems Collaborating with the wider team to ensure continuity of care and positive client experiencesWhat youll bringTo succeed in this role youll need the following qualifications:Diploma in Community Services Coordination Certificate III in Aged Care or equivalent National Police Check Current Australian Drivers LicenceWe would also love you to bring the following skills:Strong relationship-building skills and a genuine ability to connect with clients and families Empathy and respect for people from diverse backgrounds Excellent communication and customer service skills Strong organisational skills and attention to detail The ability to manage competing priorities and meet deadlines A collaborative mindset and willingness to support team outcomes Initiative and a proactive approach to problem-solvingWhy join ECHWe want you to be MORE with us.
