This is a hybrid role for someone who loves building real relationships with participants and their families while also being the kind of person who keeps compliance records and admin ticking along without missing a beat. Youll manage a caseload of aged care participants; developing and reviewingcare plans coordinating services and being their trusted point of contactwhile also handling the administrative backbone that supports great care: referrals invoicing compliance tracking and reporting.
What youll be doing:
Developing reviewing and coordinating individualised care plans in partnership with participants and their families
Building strong relationships with GPs allied health professionals and other service providers
Monitoring changing participant needs and escalating clinical risks where required
Managing referrals invoicing budgets and compliance documentation
Keeping participant files accurate audited and up to date
What were looking for:
Experience in case management care coordination or aged/disability care administration
Comfortable juggling relationship-based work with detail-focused admin
Strong communication and organisational skills
