Are you an experienced Care Partner or Case Manager looking for a varied role where you can make a meaningful difference

Resthaven is seeking two Care Partner - Relievers to join our Community Services team with one position based at our Marionand one position based at Noarlunga.

As a Care Partner Reliever you will work across our Support at Home program providing additional support across care portfolios to ensure continuity of high-quality person-centred care. You will oversee and monitor services review outcomes against individual care plans and partner with older people their families and the broader care team to support clients to remain living safely and independently in their own homes.

EssentialCriteria

• Experience in community aged care care coordination case management or a similar client-focused role.
• Program Coordination Team Leader case management experience
• An empathy and understanding of issues for older people their families and carers
• Knowledge and experience of quality systems for evidence-based care coupled with effective written and verbal communication skills
• A commitment to supporting older people to remain at home in the community
• Demonstrated ability to work in a holistic person centred model of care
• An ability to work with individuals to identify their needs develop personal goals and develop responsive packages of services
• Demonstrated computer literacy and familliarity with MS Office Suite and electronic client care systems
• A current SA drivers license

Desirable

. The following qualifications are highly recommended:

• Diploma of Nursing
• Certificate III in individual Support (Ageing) or Health Services Assistance
• Certificate IV in Aged Care Disability or Community Services
• Diploma of Community Services (Case Management)
• Diploma of Ageing Studies and services
