North and West Remote Health (NWRH) is seeking a Care Partner Administrator to provide essential administrative support to our Care this role youll help ensure clients receive timely high-quality support by coordinating appointments maintaining records managing referrals and supporting our Care Partners to deliver exceptional client outcomes.

About the Role
As a Care Partner Administrator youll provide high-quality administrative support to ensure the smooth operation of our community care programs.
Working closely with Care Partners clients families and service providers youll help coordinate services maintain accurate records and support the delivery of person-centred care.
Key Responsibilities
- Provide administrative support to Care Partners and the broader care team
- Manage client records and maintain accurate documentation within client management systems
- Coordinate appointments assessments reviews and service bookings
- Assist with referrals service requests and client correspondence
- Support data entry reporting and funding compliance requirements
- Liaise with clients families carers and external service providers
- Monitor and maintain scheduling and administrative workflows
- Assist with the preparation of reports forms and client documentation
- Support quality improvement activities and organisational processes
- Ensure confidentiality and privacy requirements are maintained at all times
About You

Essential Requirements
- Previous experience in an administration customer service or coordination role
- Excellent organisational and time management skills
- Strong attention to detail and accuracy
- Well-developed written and verbal communication skills
- Intermediate computer skills including Microsoft Office applications
- Ability to prioritise tasks and manage competing deadlines
- Professional and compassionate approach when interacting with clients and stakeholders
- Ability to work independently and as part of a team
- Current Australian Driver Licence
- Current National Police Check (or ability to obtain)
- NDIS Worker Screening Check (or ability to obtain)
- Australian or New Zealand citizenship permanent residency or unrestricted working rights in Australia
Highly Regarded
- Experience working within aged care disability health community services or not-for-profit sectors
- Experience using client management systems or electronic health records
- Understanding of aged care disability CHSP Support at Home or NDIS programs
- Experience working with people from diverse cultural backgrounds
