A supportive values-led team work that makes a genuine difference in the lives of older training professional development and career progression ResponsibilitiesLead review and maintain person-centred clinical care plans in consultation with residents families and the clinical clinical assessments referrals appointments and follow-up actions to support safe and timely care changes in residents health wellbeing and care needs escalating clinical concerns where with GPs allied health professionals families and external providers to support integrated evidence-informed accurate clinical documentation progress notes and records in line with organisational and regulatory compliance with the Aged Care Quality Standards AN-ACC requirements privacy obligations and workplace health and safety coaching mentoring and practical clinical guidance to Registered Nurses Enrolled Nurses and care team to resident and family enquiries with empathy professionalism and a solutions-focused YouCurrent Australian registration as a Registered Nurse with of 2 years experience as a Registered Nurse ideally within residential aged clinical assessment care planning documentation and AN-ACC supporting clinical education coaching or mentoring will be highly understanding of person-centred care dignity of risk and the needs of older communication organisation clinical judgement and problem-solving to build trusted relationships with residents families multidisciplinary teams and external to complete pre-employment medical National Police Check and NDIS Worker Screening nowto become an essential part of our care community with Opal HealthCare.
