  • Pay:$67.24$76.36 per hour Super Duration: 24-month contract
  • Hours:MondayFriday(37.5 hours per week)
  • Location: Surry Hills NSW

What you will be required to do:

As a Clinical Care Coordinator you will play a critical role in supporting veterans and their families to access and engage with mental health services addressing barriers and improving overall wellbeing. You will:

  • Provide short-term goal-focused clinical case management
  • Support clients with complex psychosocial needs and barriers to care
  • Conduct risk assessments care planning and brief interventions
  • Coordinate referrals and connect clients with appropriate internal and external services
  • Build strong relationships with community providers DVA and support services
  • Work collaboratively within a multidisciplinary clinical team

About You

You bring strong interpersonal skills empathy and the ability to engage with diverse stakeholders. You thrive in community-facing roles and are passionate about making a genuine difference.

Experience in community services case management veteran support or government programs will be highly regarded. Australian citizens can only apply.

To be successful in the role you will:

  • Experience in mental health case management or psychosocial support
  • Strong ability to manage complex or high-risk clients
  • Confident in risk assessment care coordination and stakeholder engagementExcellent communication skills and emotional intelligence
  • Relevant tertiary qualification in:Psychology OT or Nursing (AHPRA) ORSocial Work (AASW) ORCounselling (PACFA/ACA)
  • Working With Children and/or Vulnerable People Check (or ability to obtain)
  • Ability to obtain and maintain a Baseline Security Clearance Criminal History Check
  • Experience in case management risk assessment and complex client support
