Our Care Coordination Centre (CCC) sits at the heart of this transformation providing operational and administrative support that enables patients carers clinicians and service providers to work together seamlessly across virtual and home-based care programs.

As part of the Care at Home division the Care Coordination Centre plays a critical role in ensuring patients receive a smooth responsive and well-coordinated experience from referral through to discharge and ongoing care.

About the Opportunity

Were seeking an experienced Administration Officer to join our growing Care Coordination Centre team.

As an Administration Officer youll become a key member of a national coordination team supporting the day-to-day delivery of virtual and home-based healthcare services. Working closely with clinical teams patients carers referrers and external providers youll help ensure every patient receives a coordinated and positive care experience.

Key Responsibilities
  • Manage enquiries from patients carers referrers and healthcare providers ensuring timely and professional resolution.

  • Receive process and coordinate referrals admissions and discharges through digital care platforms.

  • Support patient onboarding orientation and engagement with virtual care technologies.

  • Coordinate scheduling and appointment management across multiple care programs.

  • Assist clinical teams with service coordination logistics equipment requests and operational support.

  • Support subcontracted service bookings and associated clinical documentation.

  • Maintain accurate patient records documentation and data within digital care systems.

  • Support billing claiming reporting and service monitoring activities.

  • Assist with workforce coordination activities rostering visibility and service delivery updates.

  • Contribute to quality improvement initiatives service development projects and continuous improvement activities.

  • Maintain and update administrative procedures and standard operating guidelines.

  • Support the delivery of an exceptional patient and referrer experience across the Care Coordination Centre.

About You

Youre an experienced healthcare administrator who is highly organised adaptable and passionate about delivering outstanding service and support.

You enjoy working collaboratively embrace new technology and take pride in ensuring processes run efficiently while helping patients navigate their healthcare journey.

You Will Bring
  • Minimum five (5) years experience in health administration customer service or a similar healthcare support role.

  • Demonstrated experience providing advanced administrative support in a healthcare environment.

  • Strong knowledge of medical terminology and healthcare processes.

  • Experience using patient management systems electronic medical records or healthcare databases.

  • Advanced proficiency in Microsoft Office Suite including Word Excel and Outlook.

  • Excellent verbal and written communication skills.

  • Strong organisational and time management capabilities with the ability to prioritise competing demands.

  • Experience working effectively both independently and within multidisciplinary and virtual teams.

  • A flexible proactive and solution-focused approach to work.

  • Commitment to continuous improvement and delivering exceptional customer service.

  • Confidence learning and working with new digital health platforms and technologies.
